The PDE-5 inhibitor Tadalafil is mainly used for the symptomatic treatment of erectile dysfunction. Other areas of application include pulmonary hypertension and benign enlargements of the prostate.

Tadalafil belongs to the group of active ingredients called phosphodiesterase-5 inhibitors (PDE-5 inhibitors), which also include sildenafil and vardenafil. Unlike the related active ingredients, Tadalafil is said to have a particularly long duration of action of up to 24 hours.

Tadalafil works by blocking phosphodiesterase-5, which in turn delays the breakdown of cyclic guanine monophosphate (cGMP) - the blood vessels dilate. This allows blood to flow more easily into the erectile tissue of the penis and enable an erection. Combined with sexual arousal, the increased cGMP levels cause a firmer and more lasting erection. Tadalafil does not have a desire-increasing effect.

The positive effect of Tadalafil on pulmonary high blood pressure (pulmonary hypertension) is also due to its vasodilating effect.

The effect of Tadalafil occurs approximately 30 minutes after ingestion, the maximum plasma concentration is reached after 120 minutes, but can also increase to 10 hours. Due to its long half-life of 17.5 hours, Tadalafil is effective for more than 24 hours.

Dosage

Tadalafil is available in tablet form in dosages of 5 mg, 10 mg and 20 mg. It is taken between 30 and 60 minutes before the desired effect is expected to occur. Eating food at the same time can delay absorption and effects. The recommended dose is 10 to 20 mg taken individually and should only be taken once a day. If more frequent use is required, daily doses of 2.5 to 5 mg are recommended, depending on tolerability.

If Tadalafil is used to treat pulmonary arterial hypertension, the daily dose is 40 mg once daily. For benign prostate syndrome, the daily dose is 5 mg.
If you have severe kidney or liver dysfunction, the maximum daily dose is 10 mg.
